Work-related accidents must be reported to the Office of Risk Management immediately. Injured employees should complete Loyola's Accident/Injury Report as soon as possible. An injured employee's supervisor must also complete a Supervisor's Accident Investigation Report and submit it to the Office of Risk Management as well.  Both forms can be found at injury.loyno.edu.
