2025 FORM 1095-C AVAILABILITY

Below is important information on how to access your 1095-C tax form, which shows the months in which you and your dependents had health coverage during 2025. 

Loyola University New Orleans will not mail the 2025 1095-C Form to you, unless you live in California, New Jersey, Rhode Island, Vermont or the District of Columbia or you request a copy.

Your form can be accessed at any time and printed via LORA Self Service by navigating to Employee > Tax Information > 1095-C Information. If you have not already selected to receive this form electronically, you can click “Change Preferences” to opt in for electronic format and once saved, the form will be available to you.

You may request a copy of your 2025 Form 1095-C to be mailed to you by contacting us through any of the following methods:
